Lumbar Facet Joint Syndrome

Lumbar facet joint syndrome occurs when the small joints in the lower spine become irritated or inflamed. This can lead to localized low back pain that often worsens with standing, leaning backward, or twisting. Physical therapy can help restore spinal mobility, strengthen supporting muscles, and reduce recurring flare-ups.

Are you experiencing low back pain that becomes worse when standing for long periods or leaning backward? Does your back feel stiff after sitting or first thing in the morning? These symptoms may be related to lumbar facet joint syndrome, a common source of mechanical low back pain.

At Rose Physical Therapy in Washington, DC, we help patients identify the cause of their back pain and restore comfortable movement with personalized, one-on-one care. Our goal is to improve spinal mobility, strengthen key muscles, and help you return to your daily activities with less pain.

What Is Lumbar Facet Joint Syndrome?

The facet joints are small joints located at the back of each vertebra in the spine. In the lumbar spine, these joints guide and control movements such as bending, twisting, and standing upright.

When these joints become irritated or inflamed, they can cause localized pain in the lower back. This condition is referred to as lumbar facet joint syndrome.

Facet joint irritation can develop from several factors, including:

• Age-related joint degeneration or arthritis
• Prolonged sitting or poor posture
• Repetitive extension movements
• Weakness in the core or hip muscles
• Poor movement patterns during lifting or exercise

Because facet joints help control spinal motion, irritation often leads to stiffness and discomfort during certain movements.

Common Symptoms of Lumbar Facet Joint Syndrome

Symptoms of lumbar facet pain are usually centered in the lower back and may worsen with certain positions.

Common symptoms include:

• Dull, aching, or occasionally sharp pain in the lower back
• Pain that worsens with standing, leaning backward, or twisting
• Stiffness in the morning or after long periods of inactivity
• Pain that may spread into the buttocks or upper thigh
• Tenderness when pressing along the lower spine

Unlike nerve-related conditions such as sciatica, facet joint pain typically does not travel below the knee.

Comprehensive Evaluation at Rose Physical Therapy

At Rose Physical Therapy, your evaluation focuses on understanding how your spine moves and identifying the factors contributing to your pain.

Your assessment may include:

• Lumbar Range of Motion Testing
Evaluating how your spine moves during bending, extension, and rotation.

• Joint Mobility Assessment
Identifying stiffness or irritation in the facet joints.

• Core Stability Testing
Assessing the strength and coordination of the abdominal and trunk muscles.

• Hip Function and Mobility
Looking for hip stiffness that may place extra strain on the lower back.

• Postural and Ergonomic Analysis
Identifying positions that may contribute to pain during work or daily activities.

• Functional Movement Screening
Observing movements such as squatting, bending, and lifting.

This detailed evaluation allows us to create a treatment plan tailored to your specific needs.

How Physical Therapy Helps Lumbar Facet Joint Pain

Physical therapy focuses on restoring healthy movement patterns and improving the muscular support around the spine.

Your treatment plan may include:

• Manual Therapy
Hands-on techniques to improve joint mobility in the lumbar spine.

• Core Strengthening
Exercises targeting the deep abdominal muscles and glutes for spinal support.

• Hip and Thoracic Mobility Work
Improving movement in nearby joints to reduce stress on the lower back.

• Postural and Activity Modification
Guidance for sitting, standing, and lifting mechanics.

• Education and Flare-Up Prevention
Strategies to manage symptoms and remain active safely.

With consistent therapy and improved movement patterns, many people experience significant reductions in back pain.
