Shoulder Labrum Tear

A shoulder labral tear occurs when the cartilage rim that stabilizes the shoulder joint becomes damaged. This injury can result from trauma, repetitive overhead activity, or shoulder instability. Physical therapy helps restore strength, improve stability, and reduce pain so patients can safely return to daily activities and sports.

The shoulder joint is a ball-and-socket joint that allows the arm to move through a large range of motion. Because the shoulder is designed for mobility, it relies on several structures to maintain stability.

One of the most important stabilizing structures is the labrum, a thick ring of cartilage that surrounds the shoulder socket. The labrum deepens the socket and helps keep the head of the upper arm bone securely positioned.

When the labrum becomes torn or damaged, it can lead to shoulder pain, weakness, and instability.

At Rose Physical Therapy in Washington, DC, our physical therapists specialize in helping individuals recover from shoulder labral tears through personalized treatment plans designed to restore movement and strength.

Common Causes of a Shoulder Labral Tear

A torn labrum can occur suddenly from injury or develop gradually from repetitive use.

Acute Traumatic Injury

Some labral tears occur after a sudden injury, such as:

• Falling directly onto the shoulder
• Experiencing a shoulder dislocation
• A direct blow to the shoulder during sports or an accident

These injuries can cause the labrum to tear as the shoulder joint is forced out of position.

Chronic Overuse

Labral tears may also develop slowly over time due to repetitive overhead motion. This is especially common in athletes or individuals who regularly perform overhead movements.

Examples include:

• Baseball or softball pitching
• Volleyball serving or spiking
• Weightlifting with repetitive overhead pressing
• Jobs requiring frequent overhead reaching or lifting

Over time, repetitive stress can create small tears that gradually worsen.

Symptoms of a Shoulder Labral Tear

A labral tear may cause several noticeable symptoms in the shoulder.

Common signs include:

• A dull or deep ache within the shoulder joint
• Pain that worsens with overhead movement or lifting
• Difficulty sleeping on the affected shoulder
• Clicking, popping, or grinding sensations during shoulder movement
• Weakness in the shoulder or arm
• A feeling that the shoulder may slip or move out of place

These symptoms may worsen during activities such as throwing, reaching overhead, or carrying heavy objects.

How a Shoulder Labral Tear Is Diagnosed

At Rose Physical Therapy, diagnosing a potential labral tear begins with a comprehensive evaluation.

Your physical therapist will typically perform:

Detailed medical history review
Discussion of how the injury occurred, when symptoms began, and which activities increase discomfort.

Movement and strength testing
Evaluation of shoulder range of motion, strength, and stability.

Specialized orthopedic tests
Specific clinical tests designed to identify signs of a labral tear or other shoulder conditions.

In some cases, imaging such as an MRI may be used by a physician to confirm the diagnosis.

How Physical Therapy Helps Shoulder Labral Tears

Treatment at Rose Physical Therapy focuses on restoring shoulder function and reducing pain through targeted rehabilitation.

Your individualized treatment plan may include:

Pain Management Strategies

Your therapist may recommend techniques to reduce discomfort, including:

• Cold therapy to reduce inflammation
• Guidance on sleeping positions that reduce shoulder stress
• Gentle movement exercises to promote circulation and healing

Restoring Range of Motion

If shoulder mobility is limited, targeted stretches and mobility exercises help restore normal movement.

Manual Therapy

Hands-on treatment techniques may be used to:

• Improve joint mobility
• Reduce muscle tightness around the shoulder
• Decrease pain and stiffness

Strengthening Exercises

Rehabilitation often includes strengthening exercises focused on:

• The rotator cuff muscles
• Shoulder blade stabilizers
• Supporting muscles of the upper back and core

These exercises help restore shoulder stability and reduce the risk of reinjury.

Personalized Home Exercise Program

Your physical therapist will provide a customized home exercise plan so you can continue making progress between therapy sessions.

Shoulder Labral Tear Surgery and Rehabilitation

In some cases, surgery may be recommended to repair the torn labrum and restore shoulder stability.

If surgery is required, physical therapy plays an essential role in recovery.

Early post-surgical rehabilitation typically focuses on:

• Reducing swelling and pain
• Gentle range-of-motion exercises
• Protecting the surgical repair

As healing progresses, therapy gradually introduces strengthening exercises and functional training to restore full shoulder function.

Recovery timelines vary depending on the severity of the tear and the surgical procedure performed.
